Hundreds of people are gathered around dancing, drinking, and having a good time. People are causally talking and laughing. Men and women from all around are having the â€Å"time of their life. † However, the lifestyle of the city, money, and connections don’t always create fulfilled, happy lives. For Daisy Buchanan, Nick Carraway, and Jay Gatsby, they are never alone but always isolated. Daisy Buchanan uses her need for attention and people to adore her most likely to cover up her fear of isolation. From the beginning Daisy has virtually been alone. Her husband Tom was not even there for the birth of their only daughter. Well, she was less than an hour old and Tom was God knows where. I woke up out of the ether with an utterly abandoned feeling†¦ † (21). She was very isolated from Tom at that point of time in their marriage. This stems her fear of being alone leads to the suspicion of Tom and Myrtle’s affair. She doesn’t have the strength to step away from Tom’s money or her fear and confront him though. She uses attention to cope with his affair. For example, she kisses Gatsby in her own husband’s house. She needs to know that someone will give and receive her attention. She lets her fear of isolation run her life. Nick Carraway is the one of the most isolated characters in The Great Gatsby. He once said that, â€Å"At the enchanted metropolitan twilight I felt a haunting loneliness †¦ sometimes (61-62). † Through this quote Nick expresses his overwhelming loneliness of city life. He is originally from the Midwest and doesn’t know many people in West or East Egg. He feels alone in the â€Å"crowd† especially at Gatsby’s famous parties when â€Å"introductions (are) forgotten on the spot† (44) and enthusiastic meeting occur between people who don’t even know each other’s names. Nick also feels alone when he realizes he had turned thirty already. He fears growing old because he sees isolation within old age. He sees the people around him and notices that no one is truly happy and as they age life doesn’t get any better. Jay Gatsby’s death confirmed his fears of isolation. Gatsby was the most well known man in West Egg and hundreds of well known, famous people showed up at his parties every night, yet â€Å"Nobody came† (143) to his funeral. Nick was just an average person just trying to work his way up to the American dream. If no one came to say goodbye to Jay Gatsby after his death, what was in store for him when he died. This mphasized his fear of growing older and being isolated. Jay Gatsby seemed like he was on top of the world from the outside looking in. He was extremely wealthy and had hundreds of people at his house at all times; however, on the inside he was just like everyone else, lonely. The love of his life married a different man when he went off to war. He had never gotten over her and always longed for her to come back to him. He isolated himself from everything that didn’t have to do with her. He made his money, bought his house, and threw his parties in hopes to impress and get her back into his arms. He was still in need of company though. He asked Nick Carraway to go to Coney Island in his car late one night. Then, when Nick said it was too late, Gatsby asked if he’d like to go swimming. Nick turned down his offer yet again. Gatsby had all the toys, money, and people in the world to interact with yet he was still lonely because he had isolated himself so much from the outside world. Even into his death, Gatsby didn’t have anyone but a few people at his funeral. He had isolated himself so much; he never made any true relationships. Throughout The Great Gatsby, Daisy, Nick, and Jay suffer from the fear or isolation of the outside world. The dream life of knowing people, being wealthy and living in the city with the upper class is as glamorous as it seemed to be for these characters. They are always around people, but always alone. In the history of the United States, the legislative branch of government has developed systems of laws which the judicial branch of government checks. Because of modernization, the constitutionality of these laws needs to be reevaluated from time to time. There have been many cases that have caused the government to amend certain laws to protect its citizens. One of the most important cases that was brought to the Supreme Court was the case of Ernesto Miranda V the state of Arizona. This case caused the government to add more to the Fourth, Fifth, and Sixth amendments to the constitution (The Supreme Law of the Land). Miranda V Arizona was a landmark case in the United States Supreme Court because it established the constitutional liberties for individuals suspected of committing crimes. In Phoenix, Arizona, during 1963, Ernesto Miranda was arrested and charged with the rape, kidnapping, and robbery of an eighteen year-old, semi retarded woman (Mount). After his arrest, Ernesto Miranda was interrogated for a session lasting about two hours, where at the end, he confessed to these charges (McBride). The police, who had not reminded Miranda of his rights before his interrogation, recorded the whole session and used his confession as the sole evidence to convict him (McBride). At the end of his trial, he was convicted of rape and kidnapping of the eighteen year-old and was sentenced to twenty to thirty years in jail (McBride). Miranda and his attorney appealed the case and had it brought to the Arizona Supreme Court with the excuse that the police obtained the confession illegally (McBride). The Arizona Supreme Court agreed with the lower court. Miranda then appealed the case to the United States Supreme Court which heard the case in 1966 (Mount). After hearing Miranda’s case the Supreme Court took a vote. The decision was five to four in favor of Miranda (McBride). The reasoning behind the Supreme Court’s decision was that the police had not properly informed Miranda of his rights before the interrogation (Miranda). â€Å"The current practice of incommunicado interrogation is at odds with one of our Nation’s most cherished principles—that the individual may not be compelled to incriminate himself. Unless adequate protective devices are employed to dispel the compulsion inherent in custodial surroundings, no statement obtained from the defendant can truly be the product of his free choice. † This quote by Chief Justice Earl Warren demonstrates that the practice of interrogation without advising people of their protection under the Bill of Rights (the first ten amendments of the United States Constitution) is unconstitutional and allows people to incriminate themselves. The requirement by the court that law enforcement officers convey these warnings to criminal suspects is taken from the Fourth, Fifth, and sixth amendment. The fourth amendment, protection against illegal search and seizure, states that police are required to have a search warrant in order to search one’s home or personal items. They also must have probable cause to obtain the warrant (Mount). The Fifth Amendment to the constitution, protection from self-incrimination, means that no one is forced to be a witness against themselves in a court of law (Mount). The Sixth amendment, the right to the assistance of an attorney, states that if the suspect charged with a crime wants a lawyer to represent him, he may call his own, or if he cannot afford one, he will be appointed one by the courts (Miranda, 1). Another reason the Supreme Court decided that not reading suspected criminals their rights is unconstitutional is because most of the time the police took advantage of people not knowing their rights (Mount). An example of this is the case Miranda vs. Arizona. The most important reason behind the courts decision in the favor of Miranda was to protect its citizens from the government (McBride). The impact of the Supreme Court’s decision was to increase the rights of citizens suspected of crime using the â€Å"Miranda Rights†. You have the right to remain silent. Anything you say can and will be used against you in a court of law. You also have the right to the presence of an attorney; If you cannot afford an attorney one will be appointed for you prior to questioning if you so desire(Miranda). These rights (Miranda rights) must be stated to a suspect prior to interrogation by a law enforcement officer (Larson). If hese rules are not followed and the officer does not read the suspect his rights before interrogation the Exclusionary act goes into effect. The Exclusionary act states that any illegally obtained evidence must not be allowed in a court of law (Miranda). Although this Supreme Court decision supported the rights of criminal defendants, there are a few exceptions to these rules. The Supreme Court ruled that â€Å"Miranda rights are not necessary prior to questioning that is reasonably prompted by a concern for the public safety. † This is called the Public Safety Exception to the Miranda Rights (Miranda). An excellent example of this would be the Supreme Court case of New York Vs. Quarles in the year 1983. In this case, a police officer received a description of Quarles and upon finding him, the officer searched him, and found that he had an empty gun holster. The officer then questioned him about the whereabouts of his gun. When Quarles responded, the officer arrested him, then read him his Miranda Rights. During his trial, the judge ruled that his statements made before his rights were read to him were allowable in court because it was an issue having to do with public safety. The court ruled in favor of New York five to four (New York). The Supreme Court also states in its rulings that the suspect must be in police custody or under police interrogation for him to have his Miranda Rights read to him(Miranda). Another case affected by the Miranda case was Illinois V. Perkins. In this case, Perkins, while incarcerated in an Illinois prison, confessed to two undercover agents that he had committed a murder. Perkins was then tried and convicted of this murder. Perkins argued that this violated his Miranda Rights and because of this he should be acquitted from the charges brought upon him. The court decided that it was perfectly acceptable because the questioning of Perkins was not done in a â€Å"police dominated atmosphere where compulsion to confess is present† (Illinois). The Supreme Court case of Miranda verses Arizona was a landmark case because it gave more rights to suspected criminals. The ruling in favor of Miranda gave criminal defendants more rights and expanded civil liberties for everyone. This case also gave the Bill of Rights more influence and power. Overall, the case of Miranda V. Arizona was very important to the history of the United States and set precedents for future cases in the judicial system.

C.S. Lewis’s, â€Å"Democratic Education,† is story that exemplifies our democratic government and how it is to be preserved through education. He gives an account of a school system where all children had equality. The equality was not that of â€Å"they were getting taught and treated the same†, but equality of the inner being. Every child should feel the same about where they are intellectually and no person should feel more or less educated and smart than the other. The account gives propositions of what society would be like in different situations depending on the other interpretations of democratic education and what prompts the positivity or negativity of having democratic education. This analysis will explore why there is strong hint that there are negative connotations about a democratic education system when the text seems to be for this particular idea. C.S. Lewis has a distinctive style of sarcasm that is found within the piece and he uses this style to make the reader realize why the idea represented is one of stupidity. He explains what would have to happen in school systems, the sources of equality, how this type of equality and democracy do not relate, how the examples given are actually not examples of democracy, and what would happen if democratic education system (despite everthing) would, in the end, hinder the boy rather than help him. Lewis begins by stating, â€Å"Until we have realized that the two things [democracy and education] do not necessarily go together we cannot think clearly about education.† Here he is bluntly stating his point of view before we even get into the argument. Postpartum Hemorrhage Essay

Postpartum hemorrhage (PPH) is a significantly life-threatening complication that can occur after both vaginal and caesarean births (Ricci & Kyle, 2009). Simpson and Creehan (2008) define PPH as the amount of blood loss after vaginal birth, usually more than 500mL, or after a caesarean birth, normally more than 1000mL. However, the definition is arbitrary, attributed to the fact that loss of blood during birth is intuitive and widely inaccurate (Ricci & Kyle, 2009). In line with this, studies have suggested that health care providers consistently underestimate actual blood loss, thus, an objective definition of PPH would be any amount of bleeding that exposes a mother in hemodynamic jeopardy (Ricci & Kyle, 2009). Currently, PPH is the leading cause of maternal mortality worldwide, and it is estimated that, over 150, 000 women, die of the complication annually (Ricci & Kyle, 2009). Causes of Postpartum Hemorrhage Excessive bleeding can occur at any time between the separation of the placenta and its expulsion or removal, and in tandem to this, there are different facets that cause PPH (Simpson & Creehan, 2008). PPH can amount from uterine atony, failure of the uterus to contract and retract after birth (Ricci & Kyle, 2009). Uterine atony is the most common cause of PPH, accounting for 70% of cases (Sheiner, 2011), and it is usually delineated by a marked hypotonia of the uterus (Simpson & Creehan, 2008). In addition, uterine atony is likely to occur when the uterus is over distended, depicted through polyhydramnios, multiple gestations, and macrosomia (Simpson & Creehan, 2008). Other factors that induce uterine atony encompass; traumatic birth, halogenated anaesthesia, lengthened labour, induction or augmentation of labour, intraamniotic infection, tocolytics, and multiparity (Simpson & Creehan, 2008). Sheiner (2011) also affirms that trauma is a significant cause of PPH, and it is typically associated with vaginal or birth canal lacerations and uterine rupture. Vaginal delivery can amount to varying asperity of vaginal, perineum-region between the genital organs and anus-, and cervix lacerations (Sheiner, 2011). Similarly, lacerations secondary to birth trauma may occur more frequently with operative vaginal birth, through the aid of forceps or vacuum (Simpson & Creehan, 2008). Another cause of PPH is retained placenta, which is primarily associated with a mean duration of the third stage of labour (8-9 minutes), and Sheiner (2011) attests that longer intervals of the third stage of labour, poses as a great risk of PPH, with double the rate after ten minutes. Further, retained placental parts interpose and interfere with uterine contractions and may either cause early or late PPH (Sheiner, 2011). In conjunction to this, coagulation disorder is also a cause of PPH. It is a rare disorder that accounts only for one percent of cases (Sheiner, 2011). Other causes of PPH include; episiotomy, uterine inversion and hematomas of the vulva, which are also associated with muscle tones, tissues, stress and thrombosis (Ricci & Kyle, 2009). Clinical Presentation and Risk Factors PPH may be divided into two presentations; early PPH, which normally occurs before 24 hours, and late PPH, which usually takes place between 24 hours and six weeks (Ricci & Kyle, 2009). Moreover, symptoms of PPH vary according to the quantity and the rate of blood loss, as well as the general condition of the mother (Simpson & Creehan, 2008). The sign and symptoms of PPH include; the apparent excessive bleeding, hematocrit-reduction of the number of red blood cells, reduced blood pressure, development of symptoms of shock and anaemia, and severe pain and swelling of tissues and muscles of the vagina, vulva, pelvic and perineum (Simpson & Creehan, 2008). Diagnosis and Assessment The principal mode of diagnosis is a differential diagnosis, and it includes a plethora of facets; bleeding from implantation site, which may be due to uterine atony, with predisposing factors such as infections, and retained placenta or abnormal placentation (Sheiner, 2011). Coagulation disorders and trauma are also essential facets considered during diagnosis (Sheiner, 2011). Conventionally, there are different methods used for the estimation of blood loss during diagnosis, and they are majorly classified as clinical and quantitative methods (Ricci & Kyle, 2009). Clinical method remains the primary means to diagnose the magnitude of bleeding and to direct interventional therapy in obstetric practice (Ricci & Kyle, 2009). On the other hand, quantitative diagnosis entails visual assessment, which is relatively, cheap, straightforward and a standard method of observation used for measurement of blood loss (Simpson & Creehan, 2008). However, the method has a lot of inaccuracy and variation from one care-giver to another, and this is usually corrected through correlations of results obtained with clinical signs (Simpson & Creehan, 2008). In light with this, assessment is also remarkably essential, and medical history available in the prenatal record can be assessed for previous bleeding disorders in order to assist the nurse in identification of risk factors for obstetrical precursors to hemorrhage (Simpson & Creehan, 2008). Therapeutic management is one of the central treatment methods used in offering remedy to PPH (Ricci & Kyle, 2009). Since 1979 illicit drug use and alcohol consumption has declined, but the widespread use of and dependence on these substances continues. Nicotine, now classified by the Food and Drug Administration as an addictive substance, is linked to more deaths annually than AIDS, cancer, and other substances combined. Alcohol is also a major social problem today. Use and abuse of alcohol are a factor in contributing to injuries and death. The cost of substance abuse is increasing in our society. Lives are lost, and loss of work productivity costs millions of dollars each year. Alcohol is a factor in 40% of all motor vehicle accidents, and alcohol and other drugs are often implicated in boating or athletic accidents.
